To save time at your initial visit, please:

1. Ensure you have contacted our office to pre-authorize your coverage with your insurance carrier.

2. Arrive 15 minutes prior to your new patient appointment.

3. Ensure you have your identification with you. Bring your Drivers License and Insurance card.

4. Wear comfortable clothing.

5. Let us know in advance if you have any questions about your upcoming appointment.
